About the role
The primary purpose of the Aged Care Clinical Assessor is to evaluate the needs of elderly clients to determine appropriate care and support services. This role involves conducting comprehensive assessments, developing personalized care plans and coordinating with healthcare providers to ensure clients receive the client centred care within a framework of our clients Mission, Vision and Values, principles and policies, Government frameworks and applicable funding criteria.
Skills and experience
- Community based role traveling throughout the Age Care Planning region
- Support the Single Assessment Service (SAS) delivering high quality in person and remote comprehensive assessments tailored to individual clients needs with a focus on wellness and reablement
- Develop strategies to connect people with appropriate services and supports in their community to further empower the client with their independence
- Conduct comprehensive assessments with client consideration to daily routine, environment, their needs and potential hazards
- Provide client centred assessments which is inclusive, ensuring equitable access for clients from diverse backgrounds and prioritising a dignified, respectful support process
- Understand the needs of elderly clients assessing each individual with sensitivity, providing culturally safe and trauma informed care
- Develop and monitor short term and long-term support plans for clients considering, assistive technology, assessment outcomes and their health, physical, social and functional needs
- Facilitate clinical referrals where clients require further specialised service by clinicians or specialised equipment
- Fostering a collaborative and respectful relationship with each client, families, other team members, community organisations, allied health providers and service agencies to ensure the best outcome for the client and their care
- Caseload management including assessment and care planning to remain within budgetary constraints of the service users designated program(s).
- Maintain high quality assessments and care planning documentation
- Manage and record feedback and complaints regarding service delivery in line with internal policies and requirements, undertaking remedy actions and escalation to address issues
- Monitor the accuracy of the electronic records of client attributable time (both direct and indirect) in the Client Management System
- Provide regular reporting of client and service status, and monitor output targets and deliverables under our funded services
- Provision of assessments undertaken using the Integrated Assessment Tool (IAT) ensuring accuracy and consistency
- Adhere to Centacare policy and procedures, best practice and clinical governance protocols aligned with Aged Care Assessment Quality Framework
- Support the Aged Care Assessor Team Leader ensuring the service is sustaining performance improvement activities including participating in audits
- Provide feedback to the business for continuous improvement opportunities
- Ability to manage your time effectively in an autonomous working environment

Requirements
- Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker
- Current unrestricted registration with AHPRA or part of a relevant professional association.
- Minimum 1 year demonstrated experience in Australia directly delivering services within aged care settings and/or to aged persons (geriatric evaluation, rehabilitation, palliative care, community nursing) including people with dementia
- Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services
- Completion of My Aged Care Assessor training would be highly regarded
- Previous knowledge or work experience in the aged care system preferred
- Experience in case management, assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services
- Demonstrated understanding of the Aged Care reform, Aged Care Standards, the sector and people who are aging in the community
- Understanding of Home Care Packages, Commonwealth Home Support Program, Short term Restorative Care Program, Transition Care Program and Residential Respite Services, IAT & Aged Care Assessment Quality Framework
- Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach.
- Local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region.
- Proficient IT skills with previous experience working across multiple platforms and Microsoft Office
