Position Overview
This facility in Canberra delivers multidisciplinary care across various hospital and community settings through several Social Work teams. The Acute Support Social Work team supports patients in critical and acute areas of The Canberra Hospital, including medical and surgical wards, the Intensive Care Unit, the Emergency Department, and maternity and pediatric inpatient wards.
The Rehabilitation Aged and Community Services team (RACS) offers integrated care in rehabilitation, aged care, and community services across multiple ACT locations, including this Hospital and community health centers. The Cancer Psychosocial Service, located in the Canberra Region, provides multidisciplinary psychosocial support to patients and families, including crisis response, counselling, and bereavement services. Social workers assess and intervene with a variety of client populations throughout their health journeys, collaborating with multidisciplinary teams to enhance client outcomes across different service specialties.
Duties
Under limited direction of the Rehabilitation Aged Care and Community Services (RACS) Social Work Manager you will perform the social work role with limited supervision and understand the issues related to health and well-being and the impact on the person and their family/carer, including adjustment to change. You will:
- Provide appropriate skilled clinical assessment and psychosocial interventions across a range of clinical areas, including individual and group work to patients and carers, with support.
- Exercise independent professional judgment on routine matters and perform novel and complex or critical tasks under professional supervision, decreasing as expertise increases.
- Liaise with patients, carers, and service providers, to enhance and improve clinical outcomes and develop partnerships for health promotion and other service activities identified in the business plan.
- Assist in the supervision and education of other staff and students. Actively participate in supervision, continuing professional development and performance management.
- Actively contribute to the implementation of clinical governance activity, quality improvement projects, research programs and health promotion in areas relevant to service.
- Actively participate in team meetings, complete clinical and administrative data collection and case records to a consistent standard.
- Practice in accordance with the professional body’s code of conduct, practice standards and organisational guidelines and legislative requirements.
- Undertake other duties appropriate to this level of classification which contribute to the operation of the organisation.

Position Requirements / Qualifications
Mandatory
- Relevant degree in Social Work qualifications and a minimum of 1 years’ experience working professionally in Social Work is preferred.
- Be registered (or eligible for registration) with the Australian Association of Social Workers (AASW)
- The successful applicant will need to be available for occasional weekend and after-hours work, with access to flex time.
- This client is leading the drive to digitally transform health service delivery in Australia through the implementation of a territory wide Digital Health Record. Computer literacy skills are required which are relevant to this role as you will be responsible for completing required documentation and becoming a proficient user of the Digital Health Record and/or other Information Technology systems; once proficient, you will need to remain current with changes, updates and contingencies.
Desirable
- Have an understanding of how the National Safety and Quality Health Service (NSQHS) indicators align with this role.
- Fulfil the responsibilities of this role in alignment to the Exceptional Care Framework, Clinical Governance Framework, Partnering with Consumers Framework and all other related frameworks.
