About the role
The primary purpose of triage delegates is to ensure that the client referral at intake is assessed and matched with an assessor who is appropriately skilled and qualified to assess according to their needs in a time manner ensuring client centred care outcomes within a framework of our clients Mission, Vision and Values, principles and policies, Government frameworks and applicable funding criteria.
Key Responsibility Areas
- Conduct triage assessment to ascertain the level of assessment required for each client
- Respond to referrals for assessment at point of intake
- Ensure clients are matched with an assessor who is appropriately skilled and qualified to assess according to their needs at time of intake
- To utilise shared technology with the centralised triage team with output goals to achieve a 90% rate of scheduling assessments at the point of intake
- Initiate prompt access for clients to our assessment services, enhancing their overall experience and expediting their journey to receiving the support they need
- Conduct thorough initial assessments to determine the client’s eligibility under the Act, deciding whether a Home Support or Comprehensive assessment is needed.
- Allocate clients to our local assessors, accessible statewide in Queensland through our Assessment Service Consortium, and mutually agree upon assessment times.
- Information gathering of the client’s Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL), ensuring a holistic understanding of their needs.
- Prioritise obtaining consent and gathering essential information, including potential requirements for Assistive Technology, Home Modification, Linking Services Support, as well as Auslan or language interpreter services.
- Conduct a risk assessment process to ensure workplace health and safety and accessibility requirements for the assessor are met.
- Creation support plans
- Connect people with appropriate services and supports in their community to further empower the client with their independence
- Understand the needs of elderly clients assessing each individual with sensitivity, providing culturally safe and trauma informed-care
- Fostering a collaborative and respectful relationship with each client, families, other team members, community organisations, allied health providers and service agencies to ensure the best outcome for the client and their care
- Maintain high quality assessments and care planning documentation
- Manage and record feedback and complaints regarding service delivery in line with internal policies and requirements, undertaking remedy actions and escalation to address issues
- Monitor the accuracy of the electronic records of client attributable time (both direct and indirect) in the Client Management System
- Provide regular reporting of client and service status, and monitor output targets and deliverables under our funded services
- Provision of assessments undertaken using the Integrated Assessment Tool (IAT) ensuring accuracy and consistency
- Adhere to our clients policy and procedures, best practice and clinical governance protocols aligned with Aged Care Assessment Quality Framework
- Support the Aged Care Assessor team ensuring the service is sustaining performance improvement activities including participating in clinical audits
- Provide feedback to the business for continuous improvement opportunities
- Ability to manage your time effectively in an autonomous working environment

Requirements
- Tertiary qualifications in a health-related discipline directly related to health aged care or related specialist area e.g as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker
- Current unrestricted registration with AHPRA or part of a relevant professional association.
- Minimum 1 year demonstrated experience in Australia directly delivering services within aged care settings and/or to aged persons (geriatric evaluation, rehabilitation, palliative care, community nursing) including people with dementia
- Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services
- Experienced in case management preferably within the home and community care sector
- Demonstrated understanding of the Aged Care reform, Aged Care Standards, the sector and people who are aging in the community
- Sound knowledge of Home Care Packages, Short term Restorative Care Program, Transition Care Program and Residential Respite Services
- Experience in navigation of the MyAgedCare platform
- Experience in service development, and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality standards
- Knowledge of IAT & Aged Care Assessment Quality Framework
- Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach.
- Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region.
- Proficient IT skills with previous experience working across multiple platforms and Microsoft Office Suite).
- Previous experience performing in a KPI environment to ensure good quality client outcomes
